INSTRUCTIONS to order the Report by postal mail:
1. Choose from three options what type of report you
desire. (See below.)
2. Send a check to the Family Tree (our parent company), P. O. Box
4311, Boise, ID 83711.
Include a note as to what option and names you have
chosen.
Include an email address. That is the
preferred way to send out the Report.

Options:
Option 1: Choose one given and one
surname only. ($3.95)
This Option is FREE for a limited
time only - but -
postage is not.
It must be sent in first to receive the Report.
However, if you're willing to receive the Report via
email,
just email us your
ancestor's name by following these instructions:
Put
a slash mark between them and capitalize the surname.
For example: William / SMITH.
Option 2: Choose up to 5 variations of
one given and one surname only. ($7.95)
Put a slash marks between them and capitalize the
surname.
For example, using William / SMITH with the known
middle name of Henry,
you may
choose: (William H./ SMITH) or (William Henry /) or (Wm. Henry /)
or (Wm. H /)
or (W. H. /) or the variety could be related to his surname:
(William /
SCHMIDT) or (William H. / SMYTH) or (W. H. / SMYTH)...
Combinations of possible variations could be
many... You can choose up to 5.
Option 3: Choose up to 5 unique names of
one given and one surname only. ($14.95)
Put a slash marks between them and capitalize the
surname.
For example:
William / SMITH
Joseph / SMITH
Harriet / GREENE
Sarah / JONES
John / JOHNSON
ATTENTION: A name with one initial only as a given name such as
(W. / SMITH)
isn't preferable simply because of the sheer volume of
possible results to be waded through!
